A Case-control Study of Skin Disorders in 643 Elderly Patients with Type-2 Diabetic in Beijing

Abstract:

ObjectiveLittle information is available about the prevalence of skin disorders among type-2 diabetic patients. The aim of our study was to evaluate the frequency and the risk factors of skin disorders in elderly patients with type-2 diabetes mellitus in Beijing, providing some reliable information for the clinical diagnose and the treatment of skin disorders among type-2 diabetic patients.Materials and methodsA group case-control study was adopted in this research. The subjects were type-2 diabetes mellitus inpatient between 1993 and 2007 in our hospital, aged 60 or above, living in Beijing.They were divided into two groups: the case group, including 634 type-2 diabetic patients with skin disorders; the control group, including 371 type-2 diabetic patients. Researcher obtained information through consulting cases of patients, including age, course of disease,diabetic complication, the type and position of skin disease, smoking, drinking, FBG,ALT, AST, BUN ,Scr ,TG ,TC ,HDL-ch ,LDL-ch and system diseases. A logistic regression analysis between the groups was also made to identify possible risk factors of skin disorders in elderly patients with type-2 diabetes mellitus in Beijing.Results1. The overall prevalence of one or more identifiable skin conditions was 63.08%. The most common skin conditions were infections (45.90%), eczema (29%), pruritus (22.56%), allergia skin disease (5.99%), non-Candidal intertrigo (3.15%), psoriasis (1.73%), pigmentary purpura (1.42%), prurigo (0.79%), bullous pemphigoid (0.79%), granuloma annulare (0.61%). 2. A logistic regression analysis demonstrated that significant predictors for skin disorders among type-2 diabetic patients included age (OR=1.07, 95%CI:1.05-1.21), FGB (OR=1.01, 95%CI: 1.03-1.09), hyperlipemia (OR=2.77, 95%CI: 2.05-3.75), diabetic complication (OR=1.40, 95%CI:1.10-1.99).3. A logistic regression analysis demonstrated that significant predictors for mycotic infection among type-2 diabetic patients included age ( OR=1.06, 95%CI: 1.03-1.09), FBG (OR=1.04, 95%CI:1.13-1.97), hyperlipemia ( OR=3.01, 95%CI:2.08-4.38),diabeticcomplication(OR=1.57,95%CI: 1.02-2.39). Significant predictors for eczema among type-2 diabetic patients included age(OR=1.01, 95%CI:1.02-1.32),FBG (OR=1.08, 95%CI:1.10-1.49), hyperlipemia (OR=2.08, 95%CI:2.05-4.37),diabeticcomplication(OR=1.07,95%CI: 1.08-1.63). Significant pred-ictors for pruritus among type-2 diabetic patients included age (OR=1.11, 95%CI: 1.07-1.44),FBG(OR=1.03, 95%CI: 1.11-1.95),smoking(OR=1.51, 95%CI: 1.98-2.46), hyperlipemia (OR=3.82, 95%CI:2.45-5.96), diabetic complication (OR= 1.02, 95%CI: 1.59-1.73).Conclusion1. The most common skin disorders in type-2 diabetic patients were infections, eczema, pruritus.2. Skin disorders in type-2 diabetic patients with the following risk factors, including age, fasting blood glucose, hyperlipemia and diabetic complication.3.Mycotic infection and eczema in type-2 diabetic patients with the following risk factors,including age, fasting blood glucose, hyperlipemia and diabetic complication. Pruritus in type-2 diabetic patients with the following risk factors including age, fasting blood glucose, smoking, hyperlipemia and diabetic complication.
