The Research between Endothelial Function and Metabolic Disorders in the Continuous Continuous Ambulatory Peritoneal Dialysis Patients
Objective:In order to investigate the relationship between Hypoalbuminemia and Endothelial function in CAPD patients.Methods:In this cross-sectional study,95 clinical stable patients(M44/F51)on CAPD in our center were included.Endothelial function was estimated by endothelial-dependent flow-mediated dilatation(FMD)of the brachial artery.Volume status was evaluated by extracellular water normalized by individual height(nECW),which was assessed by Bioelectrical impedance analysis.Some biochemical indices were determined with standard methods.Results:The FMD in Hypoalbuminemia group were significantly lower than that in normoalbuminemia group(5.59±2.77 vs.8.71±4.11,P=0.011).FMD was positively correlated with Alb(r=0.287,P=0.005)and negatively with serum fast glucose level (r=-0.274,P=0.008),nECW(r=-0.444.,P=0.001),body mass index(BMI)(r=-0.216,P=0.036), sCRP(r=-0.242,P=0.024).No correlation was found between FMD and other variables.In multiple stepwise regression analysis,Alb(β=0.295,P=0.002),nECW(β=-0.395,P=0.001) and serum fast glucose level(13=-0.251,P=0.007)were independent determinants of FMD (adjusted R~2=0.339 for this model).Conclusion:There was positive correlation between serum albumin levers and FMD, and Hypoalbuminemia was one of the major risk factors of the endothelial dysfunction.Endothelial dysfunction may be also one of the links between Hypoalbuminemia and high cardiovascular mortality. Objective In the present study,we examined whether the visceral fat is related to impaired endothelial function in CAPD patients.Methods Eighty-eight PD patients were enrolled in this cross-sectional study.Overall fat ratio and visceral fat mass were measured by multiple-frequency bioelectrical impedance analysis(BIA).Endothelial function was evaluated by brachial artery flow-mediated dilation(FMD).Result 44.3%of these PD patients were overweight or obesity.Fat mass distribution was associated age(r=0.267,P<0.05),the overall fat ratio of male patients are lower than female patients\’(20.81 vs 34.07,P<0.01),and the visceral fat mass of male patients are higher than female patients\’(12.11 vs 6.64,P<0.01).The FMD value of diabetic PD patients are lower than non-diabetic PD patients\’(5.33 vs 7.66,P<0.05).Multiple regression analysis showed visceral fat mass and glucose load were the determinants of FMD(adjusted R~2=0.180,P<0.01).Conclusion Visceral fat mass is related with age and sex of the CAPD patients,the FMD value of male and diabetic patients is lower than the female and non-diabetic patients\’. Visceral fat mass was the independent predictor FMD in PD patients.
