The Relationship between Inflammatory Cytokines and the Left Ventricular Remodeling with Acute Myocardial Infarction

Abstract:

Objective:To investigate the relationship between TNF-α、IL-6、MMP-9 and the left ventricular remodeling in patients with acute myocardial infarction.Methods:1.the gene and protein of IL-6、TNF-α、MMP-9 were examined by RT-PCR and ELISA respectively in 68 patients with AMI in the same times(one day,7th day,14th day after acute myocardial infarction).30 healthy volunteers(composed of 12males and 18 females) the same ages with patients were assigned to control group.2.LVEF,LVDd,IVSd,D1/LA4C、D2/LA4C、LVESV and LVEDV were examined by ultrasonic echocardiogram.%PLM were calculated by using Penn-Cube.Pulse Doppler evaluating left ventricular contractive and diastolic function(AFF、E/A、LVEF (%)).Ultrasonic echocardiogram were examined in 7th day、14th day and 21th day after acute myocardial infarction.Result:1.Gel electrophoresis(RT-PCR) and ELISA results display the expression of IL-6、TNF-α、MMP-9:1.1 experimental groups have significant difference compared with control group(P<0.05)at the same time;1.2 IL-6、TNF-α、MMP-9 were expressed in time-dependence manner.the expression of IL-6、TNF-α、MMP-9 in 14th day is significantly higher than in 7th day and in 1st day(P<0.05) in experimental groups.2.ultrasonic echocardiogram results display LVEF,LVDd,IVSd,D1/LA4C、D2/LA4C、LVESV、LVEDV and%PLM:1.1 Experimental groups have significant difference compared with control group(P<0.05)at the same time;1.2 LVEF,LVDd,IVSd, D1/LA4C、D2/LA4C、LVESV、LVEDV and%PLM have significant difference in 7th day than in 14th day and in 21th day(P<0.05) in every experimental groups.the left ventricular remodeling can be seen after the one week acute myocardial infarction,with left ventricular leading to ball-shape and bigger cubage and whole and local contractive and diastolic function decreased.In 21th day the percent of the left ventricular remodeling in patients with acute myocardial infarction is 45.45%.3.the levels of IL-6、TNF-α、MMP-9 have positive correlation with%PLM (r=0.930,P=0.001)、(r=0.464,P=0.001)、(r=0.937,P=0.001)respectively. Conclusions:1.The levels of IL-6、TNF-α、MMP-9 were significantly higher in patients with acute myocardial infarction 2.they were closely related to the severity of acute myocardial infarction and reflected the development of acute myocardial infarction,By detecting IL-6、TNF-α、MMP-9 we can judge the degree of acute myocardial injury 3.There was a positive correlation between the increasing degree of IL-6、TNF-α、MMP-9 and left ventricular remodeling.
