Study of PWI to the before and after TACE of Primary Hepatic Carcinoma
Objective:The aim of this study was to expore the clinical value of perfusion weighted MR imaging(PWI) to the before and after transcatheter arterial chemoembolization(TACE) of primary hepatic carcinoma(PHC),by analysising the charater of perfusion weighted MR imaging and the imaging appearance at perfusion imaging.Materials and Methods:63 cases of collected primary hepatic carcinoma were grouped into two:of these cases,MRI routine Plain scan、enhancement and PWI were performed in 32 cases(groupe one) for the first TACE before 2~3days and after 1 month of TACE.The perfusion parameters was calculated by slope analysis method depending on the appearance of the lesion;perfusion imagings were reconstructed for every patients.Other 31 cases(groupe two)were reexaminated MRI after TACE, performed MRI routine Plain scan、enhancement、PWI and DSA,Respectively;to judge the Sensitivity、specificity、accuracy rate of residual area through comparing PWI and DSA imagings.Result.63 cases of primary hepatic carcinoma after TACE,22 cases out of 32,the TSC fluctuation range decreased;7cases of TSC was straight curve,but before TACE its quickly rise;3 cases of TSC was no changed before and after TACE.20cases out of other 31 cases,the TSC quickly rise;11 cases of TSC was straight curve.HAP of the primary hepatic carcinoma showed obviously different degree high perfusion, liquefaction necrosis area was no perfusion;PVP of carcinoma was aparently uneven low perfusion;of these cases,18 cases of the lesions were completely filled with iodized oil,it showed no perfusion on HAP and PVP;22 cases,the blood perfusion of residual area was decreased after TACE;3 cases,the blood perfusion of the lesions was no apparently changed.Besides,taking DSA for standards,to judge the sensitivity、specificity、accuracy rate、negative predict value、positive predict value of residual area were 95.2%、100%、96.9%、90.9%、100%respectively, having no significant statistics,compared with DSA.Conclusion:Perfusion weighted MR imaging in primary hepatic carcinoma have no wound、fast、safety、economical and compared with DSA,have no significant statistics.Its a functional examination method,not only can display”anatomic structure,but also accurately reflect the hemodynamic change of lesions,which is very useful for the assessment of hemodynamic before and after TACE.in order to guide clinical treatment.
